A 21-year-old woman consulted an obstetrician 9 weeks into her third pregnancy with progressive nausea and vomiting. A diagnosis of hyperemesis gravidarum was made.She became confused one month later and was admitted to hospital. On examination there was restriction of conjugate gaze in all directions but especially on right lateral gaze. She had visual loss. Horizontal and vertical nystagmus was present. All her reflexes were absent. A possible diagnosis is
